PUBLIC OFFER AGREEMENT (ACCESSION)
on the provision of educational services for advanced training
LIST OF DOCUMENTS
- A completed and signed APPLICATION FORM to the PUBLIC OFFER (ACCESSION) AGREEMENT on the provision of educational services for advanced training.
- Photocopies (scanned copies) of the following documents:
- passport (1, 2 pages and place of registration (for ID card Extract from the Unified State Demographic Register on registration of place of residence);
- taxpayer registration card;
- photo for documents with a picture of the listener, which corresponds to the reached age (size 3×4 cm.)
- state-recognized document on the received degree of higher education / educational qualification level;
- payment receipt indicating the last name, first name, patronymic of the listener and “advanced training (direction of training)” in the purpose of payment.
Important !!!
One e-mail is formed and sent with high-quality photos / scanned copies in .jpg format to the e-mail address ipe@osau.edu.ua
Be sure to indicate in the email:
- in the subject – “Courses” and indicate your last name, first name, patronymic in full (for example, “Courses Pavlenko Vasily Petrovich”);
- in the text of the letter – full surname, first name, patronymic, advanced training program (indicate the direction of training.)
